Anesthetic management of pygopagus conjoined twins: techniques to evaluate cross-circulation

چکیده
Given the associated intricacies associated with surgery and anesthetic management, the care of conjoined twins requires a multidisciplinary approach. Due to the complexity of these patients, anesthetic care may be required for radiologic imaging, placement of tissue expanders, insertion of permanent or temporary vascular access devices, and definitive separation surgery. We present a case of pygopagus conjoined twins with multiple anesthetic encounters, including definitive separation surgery. The perioperative management of such patients, with emphasis on the shared circulation and airway management, is discussed.

Successful Surgical Separation of Thoraco-Omphalopagus Symmetrical Conjoined Twins in Iran: Two Case Reports
Conjoined twins are derived from the division of a single fertilized ovum; a phenomenon accompanied with multiple congenital anomalies. Such twins are identical, of the same sex, and more likely to be female. Most twins die during the embryonic period, and only 18% survive longer than 24 hours. There are complex anomalies in thoraco-omphalopagus twins that makes them unlikely to live long enoug...

OBJECTIVE To summarize our experience in the anesthetic management of conjoined twins undergoing one-stage surgical separation. METHODOLOGY Medical records of conjoined twins admitted to our hospital for treatment and considered for surgical separation from 1996 to present were retrospectively reviewed. Four cases of conjoined twins underwent one-stage surgical separation under general anesth...

Conjoined twins are a rare phenomenon of a monochorionic monoamniotic twin. The incidence varies from 1 in 50000 to 1 in 100000 live births [1,2]. Female foetuses are more commonly affected with the ratio of male to female being 1:3, particularly in thoracopagus type [3]. Five types of conjoint twins are classically described, Thoracopagus, Omphalopagus, Pygopagus, Ischiopagus and Craniopagus. ...

BACKGROUND: Asymmetric and parasitic conjoined twins are rarer anomalies of monochorionic monoamniotic twins, consisting of an incomplete twin attached to the fully developed body of the co-twin. CASE PRESENTATION: A 30-year-old multigravid woman referred to maternal fetal unit due to polyhydramnios at 28th week of gestation. Sonographic examination revealed a single fetus and polyhydramnios wi...
